Please bring your child’s birth certificate and immunization records.
All students entering Kindergarten, seventh grade, or transfer students from out of state are required to have a physical examination by a licensed physician before entering these grades. The physical exam must be within six (6) months prior to the beginning of school.
All students entering Kindergarten, or transfer students from out of state, are required to have an eye examination within six (6) months prior to entering Nebraska schools for the first time.
